Miscellaneous Card & Envelope

The miscellaneous card is a single card and evelope to be used for a requested third screen, a recollection for an unsatisfactory sample, or to replace a lost second screening card.

When submitting a miscellaneous card, the infant’s original Newborn Screening Kit number must be written on the replacement card.

Please call the newborn screening program at 801-584-8256 to obtain the infant’s original Newborn Screening Kit number. This will allow proper tracking and comparison of results between the first, second, and any additional screenings.
